In general, major city stations are fully wheelchair accessible. When notified in advance, train company and station staff can help with boarding and alighting.
Modern, high speed trains usually have spaces allocated for wheelchair users (plus a companion), close to accessible restroom facilities.
Such spaces are limited though, so it’s best to book well in advance if you can.
